NDIS Assessments

Formal assessments provide evidence to the NDIS of the supports you need to achieve your goals.

NDIS Assessments

Functional Capacity Assessment

A functional capacity assessment looks at how well you’d do things like household chores and socialising, as well as how much you can do at home and in your community. The FCA looks at your physical abilities as well as any problems that might stop you from doing normal day-to-day things.

Some of the things that will be included in the FCA are:

How well you can perform daily living tasks both at home as well as outside of home
Your home environment and what modifications need to be made, or what type of assistive technology would be appropriate
Transport needs or mobility to get to and from/within the community
Social relationships or a lack thereof that support you with belonging to the community
Physical, cognitive, and/or behavioural factors affecting your ability to function including developmental needs for younger people

Once the NDIS assessment has been completed you will have a better understanding of what you need to achieve your goals or live independently from others, what support you need and what kind of allied health professionals will support you in your journey.

Supported Independent Living (SIL)

Supported Independent Living (SIL) refers to funded, personalised support services designed to help you live as independently as possible in your own home and community. SIL assessments are thorough and person-centred, focusing on your current functional abilities, lifestyle goals, and the level and type of support required to enhance safety, wellbeing, and overall quality of life.

In a SIL assessment, our allied health team carefully reviews key areas of your daily life, including:

    • Assistive equipment needs to support independence and safety
    • Home environment assessment and recommendations for home modifications
    • Personal and domestic daily living support requirements
    • Community access, mobility and movement support, and social participation needs
    • Complex care needs and ongoing support planning

This comprehensive assessment ensures your SIL supports are tailored specifically to you, empowering you to live with greater choice, control, and independence in the place you call home.

Specialist Disability Accommodation

The NDIS funds Specialist Disability Accommodation (SDA), which provides housing that has been tailored specifically to meet the needs of people with very high levels of lifelong support needs for whom standard housing cannot provide safe and/or practical environments to meet their physical, cognitive and/or sensory needs.

SDA is built and/or modified to have specialised features such as wide door access for wheelchairs, sensory-friendly environments and emergency response systems that provide greater safety and independence, therefore increasing your quality of life.

To support you through your SDA journey, our team provides a comprehensive, person-centred SDA assessment that:

Identifies your current and future functional needs (e.g., limitations on mobility, sensitivities to sensory input, and/or the need for 24/7 access to support) in accordance with SDA eligibility requirements.
Documents the current and future limitations of standard housing in order to demonstrate why the recommended housing solution(s) are considered “reasonable and necessary.”
Facilitates a discussion of your housing preferences (e.g. location, living arrangements with family/peers) to ensure that the recommended housing solution(s) are aligned with your known lifestyle choices.
Generates a detailed and evidence-based report that will support your application for SDA funding from the NDIS and your SDA housing application.

You can choose either a fully modified home or access to purpose-built SDA properties. Either way, our assessments will ensure the housing solutions we recommend are tailored to meet your individual needs so you can be empowered to
